Dog housebreaking at night change after 3 yrs.?

My Yorkie/Jack Male is 3 yrs. old and I crate trained him for one year when I got him. He goes outside and also on a pee pad (just for the 2nd duty). I bring him out to pee just before bed every night. During the day he can hold it for up to 10 hrs. Last night I brought him out at 11:00 p.m. and he peed on the pad sometime during the night. He has been doing this off and on for the past couple of months. If I forget to put a pee pad down, he will go on my bathroom carpet. Sometimes, he will choose to just go on the carpet and not the pee pad. He never goes in the house during the day whether we are home or not. Is he just getting lazy? Should I re-crate him nightly for a few weeks? This only happens about once a week.
